proposed SOLUTION 3: "EARNED SABBATICAL" (does not apply just to PTs). Defined: After working 5-7 years in the same job or field, you would get a Govt subsidized "significant break", i.e. 6 month block of "Paid 'Personal' Time Off". This needs no further explanation or justification, but if you're doubting the appeal/value to workers, send out a Survey Monkey Question on it to all PTs --though better if a mailed Paper Ballot with verifiable License numbers as 'digital' is vulnerable to hacking. This most valuable commodity of "Time", would be like a "pressure-relief valve" to workers. ...time to decompress; clean; make repairs; organize/plan/strategize one's diet, health, exercise, career, finances; take self-paced coursework; enjoy family/friends, hobby, recreation, &/or travel. Current American Culture is to Use, Suck Dry, then Neglect/ Toss employees after working years completed. ...& eventually perhaps the final solution of "Soylent Green"
proposed SOLUTION 2: DRASTICALLY REDUCE the Insane Documentation that is imposed on PTs! I mean, c'mon, ever read a Physician's, PA's documentation? To the Point. Essential. Do all those STG/LTG/"Functional" Goals REALLY matter? How's THAT for a 'Research' project?! My hypothesis between groups after 12 weeks: No difference between "Goal" and "Non-goal" groups. Why? Because they're already inherent in what Therapists do. Does a Surgeon /PA have to write STG/LTG/Funct goals? No. Does it compromise their treatment/outcomes? No. Do the Research. Then rebut Medicare & other Payors with the Findings. If a PT can 'reclaim' 10 mins on each patient by having less detailed Documentation requirements, what would That do to 'work-life' balance, morale, energy-level, outcomes?!
To reiterate, "Tasks under Time Compression", PT training should include becoming Waitstaff &/or Bartender for 3-6 months, and a 2-3 week intensive course in "Plate-Spinning" (with Different size/style of dinnerware at that). I am being serious btw. So, proposed SOLUTION 1: Better Preparatory Training ('Practical/Behavioral') --BEFORE stepping in a "clinical" role.
You painted some broad strokes of 'potential' solutions. Any particulars? Look, a big #1 is addressing 'stress', i.e. "Tasks under Time Compression". How many "leaders" at the decision-making level at APTA or PT Academia are in-touch with the day-to-day realities of being a front-line PT? Surgeries from Orthopedics, Colonoscopies, to Cataracts are becoming "assembly-line-like", and Industry & Economic Forces (e.g. lower reimbursements / inexcusable inflation) have corralled PT's into doing the same.
Great encouragement and perspective. I wonder though, is it easier to state such as an owner of ~20 clinics, being in an executive, developer, investor role? With as you say, "declining reimbursements", the inevitable consequence to PTs is "increasing patient volume". This equates to a higher likelihood for burnout, disappointment (as less Quality time with the patient; Outcomes/Patient satisfaction diminishing) and disengagement/attrition. "Work-life balance" becomes a tongue-in-cheek smirk.

Inject cortisol or refer them to Orthopaedics even not getting better refer them to China Traditional medicine still if not, refer them to pain clinic for CBT OR If not getting better, think about more advanced approach- MDT or nerve block injections

Valuable insights
Not sure what to take away from this video. Is this an alternative exercise that lessens shoulder pain or what?
It is, i experienced shoulder pain few months ago, started to alternate my regular shoulder press to the high incline press as shown in the video because it feels more comfortable and painless on my shoulder joint. My shoulder started to feel better, and i always do the high incline press for my main shoulder exercises since then, even after i no longer feel any pain or discomfort on my shoulders on regular shoulder press. Plus, i also surprised that i feel very little weight difference between shoulder press and high incline press, since i can perform high incline press with 35kg for 8 reps, and regular shoulder press for 6 reps😂
